Steps in completing
this packet: |
|
It is important that
only the linked forms below be used. For example,
we must have an official transcript from the school
that arrives in our hands in an officially sealed
envelope.
-
Print and
complete each linked documents below and
return/mail to Sullivan University within seven
working days after receiving the packet.

Before returning
the packet check for the following:
-
All
addresses are complete (i.e., previous
employers, schools, etc.).

All
transcript request forms clearly spells out
the complete name of all schools. (e.g.,
University of Kentucky, not UK)
-
Once all
forms have been completed as directed, mail
attention to
Jane LeCompte
(Administration Office)
-
If you are
on the main campus, please deliver it to the
Administrative Office.

Documents to be completed:

Your Employment
Package must contain the following six (6) items
items when submitting your Sullivan University
Employment package submission. Four of these
items are downloadable Adobe .pdf files (links
below), and you will need to provide the other two
(2) items.

Instructor Qualification
Form: Fill out the enclosed
form(s) with current information and place it in
the return envelope. Please print all
information.

Work Experiences:
Using a blank
sheet of paper, expand on the entries made on
the Instructor Qualification Form, "Work
Experience Relative to this Position" section. Provide a list
of work experience(s) that support your
qualifications for the position in which you
will be working. In the case of a faculty
member, you will provide these forms for the
area(s) you will be teaching. Make as many
copies of the employment verification form as
you need (include historical employment record
in package).

Resume:
Include an updated
resume/vitae in
your package.

Transcript Request Forms:
Fill out a transcript request form for each
institution you have attended. Print as many
copies of this form as needed. Sullivan
University will cover the cost of requesting
official transcripts; simply return the
completed transcript forms.

Certificates and
Professional Licensure: Provide a
copy of all certificates and/or professional
licenses you have earned that pertain to the
position in which you will be working. In the
case of a faculty member, you will provide these
forms for the area(s) you will be teaching.
Make as many copies of the certification and
professional licensure verification form as you
need.

Employment Verification:
Provide contact information of previous
employers for verification of employment history
as stated in the applicant’s resume.
Special
Considerations, Instructions, &
Notes:
(1)
On occasions
agencies will not release requested information to
third-party requestors (e.g., Sullivan University).
If this occurs, the applicant will be informed and
is responsible for procuring the required
information directly form the corresponding
agency/agencies. Once obtained, the applicant is
required to forward the information to Sullivan
University.
(2) It is the
responsibility of each applicant to complete
each area of the information requested where
applicable. DO NOT ALTER ANY PORTION OF
THESE FORMS. All transcripts must be
official transcripts from the granting
institution; unofficial copies will not be
accepted.
(3) Do not use the following terms
as responses when
filling out the forms:
Use of these terms will result in
the package being returned to you for clarification.
Always provide the specific information requested
even when it appears redundant.
